Endovenous Laser Ablation for Varicose Veins
Endovenous laser ablation, or EVLA, treats certain malfunctioning leg veins using laser energy delivered inside the vein. It is a minimally invasive option for addressing venous reflux, the backward blood flow that can contribute to varicose veins and symptoms such as aching, heaviness, and swelling.

What Is Endovenous Laser Ablation?
EVLA uses a thin laser fiber to heat and close a problem vein. Ultrasound guides the fiber into position through a small puncture or opening in the skin. After treatment, blood returns through other functioning veins, and the closed vein gradually shrinks as the body heals.
EVLA is generally performed as an outpatient procedure using local anesthesia. It does not involve removing the vein through traditional vein-stripping surgery.
What Does EVLA Treat?
EVLA treats reflux in suitable superficial veins, including the great or small saphenous vein. These veins run beneath the skin and may contribute to visible varicose veins when their valves do not function properly.
Treating the underlying reflux may help relieve symptoms, but it does not necessarily remove every visible vein — some bulging branches may need additional treatment. EVLA also differs from laser treatment applied to the skin’s surface; small spider veins usually require a different approach.
Who May Be a Candidate for EVLA?
Laser ablation may be considered when an evaluation identifies reflux in a vein that can be treated safely with a laser fiber. Your clinician will consider:
- Your symptoms and how they affect daily life
- Ultrasound findings, including the location of reflux
- The vein’s size, depth, and shape
- Your medical history and previous vein treatments
- Any history of blood clots or other circulation problems

What Happens During the Procedure?
- Ultrasound guidance — ultrasound identifies the vein being treated and guides the placement of the treatment equipment.
- Access to the vein — the skin is numbed, and the clinician introduces a thin tube and laser fiber through a small puncture or skin opening.
- Local anesthesia around the vein — numbing fluid is placed around the vein (tumescent anesthesia), helping control discomfort and protecting nearby tissue from heat.
- Laser treatment — the clinician delivers laser energy while withdrawing the fiber along the treatment segment, and the heat causes the vein to close.
- Dressing and aftercare — the fiber is removed, and a dressing is placed over the access site, with instructions about walking, compression, and follow-up care.
Treatment often takes about an hour, although the full appointment may take longer.
EVLA vs. Radiofrequency Ablation vs. Varithena
These treatments close selected problem veins using different methods. The choice depends on the veins involved, your health history, and the benefits and risks of each option — recovery also varies with the procedure and any additional treatment performed.
- EVLA — a laser fiber delivers heat inside the vein; typically uses local and tumescent anesthesia.
- Radiofrequency Ablation — a catheter delivers heat generated by radiofrequency energy; typically uses local and tumescent anesthesia.
- Varithena — prescription foam is injected into the vein to close it without heat; does not require tumescent anesthesia, though local numbing may be used at the access site.

Recovery After EVLA
Most patients go home the same day, and walking is usually encouraged after treatment. Bruising, tenderness, or a pulling sensation along the treated vein can occur during recovery. Your care instructions will explain:
- Whether to wear compression stockings and for how long
- How to care for the access site
- When to return to work, exercise, and other activities
- Any restrictions on travel or prolonged sitting
- When to return for follow-up assessment
Light daily activity and strenuous exercise have different recovery requirements — follow your clinician’s instructions rather than assuming all activities can resume within a few days. Before your appointment, ask whether you will need someone to drive you home.
Results and Follow-Up
EVLA is intended to close the refluxing vein and help reduce the symptoms associated with it. Improvement varies between patients, and visible veins may change more gradually than symptoms. Additional treatment, such as sclerotherapy or ambulatory phlebectomy, may be recommended for remaining veins.
A treated vein can sometimes fail to close completely or reopen. New varicose veins can also develop over time. Follow-up helps assess the result and determine whether further care is needed.
Risks and Side Effects
Possible side effects include bruising, tenderness, skin discoloration, and inflammation along the treated vein. Less common complications include infection, skin burns, nerve irritation or numbness, and blood clots. Your clinician will discuss your individual risks before treatment.
Contact your care team promptly for worsening pain, significant swelling, spreading redness, or fever. Sudden shortness of breath or chest pain requires emergency care; call 911.
Is EVLA Covered by Insurance?
Coverage may be available when treatment meets your plan’s medical-necessity requirements. Symptoms, ultrasound findings, and prior treatment may be considered. Documented reflux does not automatically guarantee coverage — confirm authorization requirements and expected out-of-pocket costs before treatment.
